Advance Care Planning Workshop for Health Professi

Target Group - Registered Nurses, Enrolled Nurses & Allied Health Staff
Advance Care Planning is a process of planning for future health and personal care. A person’s values and preferences are made known so that they can guide decision-making at a future time when the said person cannot make or communicate their decisions due to lack of capacity (Australian’s Health Minister’s Advisory Council)
It is a requirement that those attending complete the first four online learning modules on the Advance Care Planning Australia website prior to the ACP Training. We ask that participants bring the four certificates of completion with them.
Module 1. ACP Introduction
Module 2. Planning Conversations
Module 3. Decision Making – The Legal Implications
Module 4. Implementation               
Please note: There are a total of 9 online modules. It is strongly recommended that participants complete all modules, starting with those applicable to your workplace.
